NDIS
Achieve Your Goals with Compassionate NDIS Supports
At Freedom Focus Home and Care Services, we believe that living with disability should never stand in the way of living well. As a committed NDIS provider, we deliver personalised support services that give participants the tools, confidence, and opportunity to pursue the life they want.

We take the time to understand who you are, what drives you, and where you want to go before we put together a support plan that actually works for your life. Whether you need a hand at home, support out in the community, or someone to help you navigate a big change, our team brings genuine care and consistency to every interaction.

Understanding the NDIS
The National Disability Insurance Scheme (NDIS) is the Australian Government’s national framework for funding and supporting people living with permanent and significant disabilities. It gives eligible participants access to the services and supports they need to live more independently, connect with their communities, and work towards personally meaningful goals.

The NDIS puts the individual at the centre. Participants have real choice and control over the supports they receive and the providers they work with. Funding can cover a broad range of reasonable and necessary supports that build skills, grow independence, and make community participation more accessible.

How the NDIS Works
NDIS funding is provided directly to eligible participants based on their individual goals, circumstances, and support requirements.

Once approved, you receive an NDIS plan that sets out your funded supports. These are designed to help you reach specific outcomes, whether that is greater independence at home, stronger daily living skills, access to employment, more active social participation, or better overall wellbeing.

You decide how your funding is managed and which providers you engage, so your supports stay aligned with your priorities and the way you want to live.

Are You Eligible for NDIS?
At Freedom Focus Home and Care Services, we are always happy to help you work out whether the NDIS is the right fit for you. Here is a quick overview of the general eligibility criteria.

You may be eligible for the NDIS if you:

  • Are under 65 years of age at the time of applying
  • Are an Australian citizen, permanent resident, or hold a Protected Special Category Visa
  • Have a permanent or significant disability that affects your ability to carry out everyday activities
  • Require ongoing support due to your disability, now and into the future

Getting Started with NDIS Supports
Our onboarding process is straightforward and designed to get you the support you need without unnecessary delays. Here is what to expect.
  • Step 1: Let’s Have a Conversation
Contact our team to tell us about yourself, your goals, and your NDIS plan. We listen carefully and answer any questions before suggesting a path forward.
  • Step 2: Building Your Support Plan Together
We collaborate with you, your family, and your support network to develop a personalised plan that genuinely reflects your goals, preferences, and funding arrangements.
  • Step 3: Confirming Your Agreement and Finding Your Match
We finalise your service agreement and thoughtfully match you with support workers who suit your personality, communication style, and specific support needs.
  • Step 4: Your Support Begins
Services commence, and we stay actively involved through regular check-ins, ongoing guidance, and flexible adjustments to keep you moving forward.
Putting Together Your NDIS Plan
Your NDIS plan is a living document built around your goals, your support needs, and where you want to be in the future. At your planning meeting, you will talk through your day-to-day challenges, the supports already in place, your longer-term goals, and what you need to live more independently.

The more clearly you can describe your situation and what you want to achieve, the better your plan will reflect what you genuinely need. We can help you prepare for this conversation.

Making the Most of Your NDIS Funding
Once your plan is approved, you can start drawing on your funded supports. Depending on how your plan is managed, you may self-manage your funding, work with a plan manager, or have the NDIA manage it for you.

Knowing your funding categories and how to use them wisely means you can get maximum value from your plan and make steady, real progress toward your goals. Our team is happy to talk you through this.

When Your Needs or Goals Change
Life moves, and your NDIS plan can move with it. If your circumstances shift, your goals evolve, or you need a different level of support, your plan can be reassessed and updated to stay relevant to where you are now.

Major life changes, new goals, or a need for additional supports are all good reasons to discuss a plan review. Freedom Focus Home and Care Services will help you understand your options and prepare for the conversation with confidence.
